Job Description
The Family Advocate (Case Manager) plays a vital role in bridging communication and support between youth, families, and community systems. This position focuses on juveniles participating in the Strengthening Families Program and Teen Pregnancy Prevention Program, ensuring families receive the guidance, advocacy, and resources they need to achieve long-term success.
The Family Advocate will serve as a liaison between CPS social workers, probation officers, school staff, and families to monitor progress, support compliance with community service requirements, and connect participants to appropriate community resources. This role also includes providing case management and mentoring to teen parents, helping them re-engage in education, explore vocational or higher education opportunities, and develop life and parenting skills.
Key Responsibilities
Serve as a liaison between families, social services, probation officers, and community partners to coordinate care and share progress updates.
•Provide individualized case management and advocacy for youth and parents in the Strengthening Families and Teen Pregnancy Prevention programs.
•Monitor and document participant progress toward goals, including completion of community service hours and other program requirements.
•Assist teen parents with re-enrollment in school, GED programs, or vocational/job training opportunities.
•Connect families to community resources such as housing, childcare, transportation, counseling, and employment support.
•Conduct home visits and follow-up meetings as needed to assess progress and provide encouragement.
•Collaborate with program facilitators to ensure smooth communication between the Strengthening Families Program and Teen Pregnancy Prevention Program.
•Maintain accurate case notes, reports, and data entry for grant and program compliance.
•Participate in staff meetings, trainings, and community outreach events as required.
Qualifications:
•Bachelor’s degree in Social Work, Human Services, or related field (Master’s preferred)
•2+ years of experience working with at-risk youth or family support programs
•Strong communication, case management, and community resource navigation skills
•Valid NC driver’s license and reliable transportation required
•Bilingual (English/Spanish) preferred
Schedule & Compensation:
•Part-time, flexible hours (15–25 hours/week)
•Hybrid schedule – combination of remote work and in-person meetings in Salisbury, NC
•Contract-based compensation, commensurate with experience
